To schedule an appointment by email:

Please send us an email with your name, a telephone number, the time(s) and date(s) you would like for your appointment as well as what treatment you are requesting. We will check our schedule and email you back what is available at or around the date and time you requested.Your requested appointment is not booked unless we either email you or call you to confirm availability, so please don't come in if you did not receive confirmation from us. Medical questions are never adequately answered by email, your unique concerns are best answered in person. Questions other than appointment times can not be answered by contacting us using this email.

If you have an appointment with us and need to cancel, you can email us the date and time of the appointment you wish to cancel, and also let us know if you need to reschedule.
